Budget for Life Application

Please submit an online application in its entirety, if a question does not apply to you, please enter N/A.

The following documents MUST be attached to the application:

  • Applicant photo ID
  • Current signed lease or mortgage statement in your name
  • One month’s proof of income to include: (2) recent pay stubs and any other source of income you receive (ex. food stamp award letter, child support, disability award letters, parttime job, etc.)

What happens once I fill out the application?

A case manager will review your application to determine if you meet our minimum criteria. If you do not meet our minimum criteria, you will receive an email from the case manager stating you are not eligible. If you meet the minimum criteria, you will receive a call from the case manager to schedule a virtual interview and additional information will be requested for the interview.

The Budget for Life Program is a highly competitive program, submission of an application does not gurantee acceptance into the program.
